Main Street Waterloo Volunteer Waiver & Release
In exchange for the opportunity to participate in volunteer activities with Main Street Waterloo (“Volunteer Activities”), I, the undersigned Volunteer (or Parent/Legal Guardian if the Volunteer is under 18), agree to the following:
1. Release of Liability – I release and agree not to sue Main Street Waterloo, its officers, directors, employees, sub-contractors, sponsors, agents, and affiliates (“the Foundation”) from any present or future claims that may arise due to property damage, personal injury, or wrongful death resulting from my participation in Volunteer Activities, regardless of where or when they occur.
2. Assumption of Risk – I understand that volunteering may involve certain risks, including serious injury or death. I acknowledge these risks and voluntarily participate with full knowledge of potential dangers. I accept all risks associated with my involvement.
3. Indemnification – I agree to hold harmless and indemnify the Foundation against any claims, liabilities, damages, or expenses resulting from my participation in Volunteer Activities.
4. Insurance Acknowledgment – I understand that the Foundation does not provide insurance coverage for me or my heirs and assigns. I confirm that I am responsible for any medical expenses or losses incurred while volunteering.
5. Health & Fitness – I certify that, to my knowledge, I am in good health and do not have any physical conditions that would prevent me from safely participating in Volunteer Activities.
6. Legal Agreement – I understand that this waiver is a legal contract that affects my rights. If any part of this document is found to be invalid, the remaining provisions will continue in full force and effect.
By signing below, I confirm that I have read, understand, and voluntarily agree to the terms of this waiver.
